Why we did this

When Lilly confirmed the September hike, we knew patients needed clarity fast. We then estimated where September prices might land (our model here), tracked the drip-feed of pharmacy announcements in the run-up (latest updates here), and saw how providers adjusted right before the deadline (coverage here). This article brings it all together by looking at the annual cost of treatment — not just single doses.

How we crunched the numbers

To make comparisons fair, we used a standard journey based on Lilly’s official titration schedule:

• Weeks 1 to 4: 2.5mg once weekly
• Weeks 5 to 8: 5mg once weekly
• Weeks 9 to 12: 7.5mg once weekly
• Weeks 13 to 16: 10mg once weekly
• Weeks 17 to 20: 12.5mg once weekly
• Weeks 21 to 52: 15mg once weekly

This is a simple and transparent model. Not every patient will follow this exact path, as everyone’s journey is unique, but it provides a consistent way to compare costs.

For each pharmacy, we calculated the annual cost of this journey. We then worked out an average yearly cost and set this as an index of 100. Each provider’s cost was compared against this index. Some pharmacies appear twice because they offer both subscription and non-subscription (or member and non-member) pricing.

The cost breakdown by dose

When we look at how much each dose contributes to the total annual cost, the picture is striking:

• 2.5mg: 5%
• 5mg: 5%
• 7.5mg: 7%
• 10mg: 7%
• 12.5mg: 8%
• 15mg: 68%

That means almost 70% of your annual spend comes from the 15 mg dose alone, and when you add 10 mg, 12.5 mg and 15 mg together, they represent 83% of the total cost.
